EVA Support welcomes all feedback and complaints and encourages anyone who has something to say to contact us so we can strive to improve.

If you wish to offer any feedback or complaints, please don’t hesitate to fill in the form. We will investigate any complaint(s) you have made in a timely manner and advise you in writing of the outcome.

How to make a compliant

Please talk to the person involved or the manager. 

You can ask your advocate, community visitor or family member to speak with the manager.

If you need an interpreter when providing feedback, ask to speak to one of our staff or the manager.

Alternatively, you can call us to discuss the matter or download our feedback form and return it to us by post or email.

In accordance with the Complaints, Compliments & Suggestions Policy of NDIS, we aim to facilitate the fair, prompt and confidential resolution of all complaints.
